Working as part of a multidisciplinary team, the RN provides front-line outreach as well as onsite nursing care to people living with HIV/AIDS who face barriers to accessing treatment and are significantly marginalized by poverty, drug addiction, mental health and housing instability. The RN proactively locates clients, engaging with them at street level, and works to retain them in HIV care utilizing intensive case management and care coordination in partnership with the client and their support network. This role also includes organizing monthly treatment clinics in partnership with the Infectious Diseases Care Program.

REQUIRED QUALIFICATIONS

  • Postsecondary diploma (RN) or degree (BScN) from a recognized college or university
  • Current registration with the College of Nurses of Ontario
  • 3-5 years of nursing experience in a community setting or combination of community and hospital / public health
  • Awareness and understanding of harm reduction, social determinants of health and health promotion
  • Ability to work with diverse communities with respect, sensitivity, creativity, innovation, cultural understanding and non-judgement
  • Excellent written and oral communication skills
  • Ability to maintain complete and accurate records in accordance with the Health Centre’s policies
  • Strong computer skills, including the use of MS Office Products and Electronic Medical Records (Telus Practice Solutions Suite currently in use)

PREFERRED SKILLS

  • Strong knowledge of HIV/AIDS, and other infectious disease practice; demonstrated phlebotomy skills 
  • Experience in street level outreach, assertive engagement and motivational interviewing skills an asset
  • French and other language skills are considered an asset in supporting the communities we serve
