Job Description

TransForm Shared Services Organization (TransForm) is a leading, premier healthcare not-for-profit organization that was formed in January 2013 and consists of managing three divisions which support five Erie-St. Clair hospitals, namely:

i.Specialty Information Management/Information Technology services;

ii.Supply chain and specialized contract services;

iii.Transformation Care Team which incorporates a Project Management Office to address local, regional and provincial health priorities

All divisions work with various health service providers and government officials to lead transformational change. Specifically all divisions are focused on providing value, improving the patient experience, achieving better health outcomes and utilizing health resources more effectively. All three divisions work to support our customers in the delivery of exceptional patient care by ensuring efficiencies, service excellence and cost savings. TransForm is currently owned by the five Erie-St. Clair hospitals and is governed by a Board of Directors comprising of senior executives from each hospital, namely: Bluewater Health, Chatham Kent Health Alliance, Hotel Dieu Grace Healthcare, Leamington District Memorial Hospital and Windsor Regional Hospital as well as five independent Directors.

PREAMBLE:

Connecting South West Ontario (cSWO) is funded by eHealth Ontario with the goal to implement a Regional eHealth Program that will make an individual’s health information from across the continuum of care available in a timely and secure fashion at any point of care. The cSWO Program is foundational to eHealth Ontario’s commitment to integrate electronic health information for all Ontarians.

London Health Sciences Centre (LHSC) is the delivery partner to eHealth Ontario, with accountability for the cSWO Program Management. There are four Change Management and Adoption Delivery Partners (CM&A DP) that are responsible for the deployment of cSWO Program services. Their role is to work with local health service providers to facilitate the business and technical adoption of cSWO electronic health regional Program solutions and standards into the regular delivery of care.

In Erie St. Clair LHIN (ESC LHIN), the cSWO Program is being delivered by TransForm Shared Service Organization. This exciting multi-year Program includes the planning, implementation, adoption and evaluation of key components of the electronic health record and is focused on engagement with local health service providers to leverage existing provincial, regional and local solutions, including the Regional Clinical Viewer (ClinicalConnect).

POSITION SUMMARY:

As Change Management Site Coordinator for TransForm SSO the role will be responsible for implementation of the cSWO Program in ESC LHIN. Responsible for ensuring standardized change management documentation and processes are adhered to for all implementations across the ESC LHIN.

This position is responsible for working with cSWO Change Management Team to ensure the successful adoption of cSWO deliverables across the ESC LHIN.

JOB RESPONSIBILITIES:
 
  • Works with the project team and the end users (physicians/hospitals/NP’s/clinical office staff) to implement the change management and adoption strategy of the cSWO solution
  • Follow a set of comprehensive standardized documentation and change management process across the ESC LHIN
  • Directly report to the Change Management Lead; provide regular updates on new and upcoming implementations and outstanding support issues
  • Validate new EMR/hospital connections by conducting UAT and Pilot testing
  • Coordinating an implementation with the end users across different healthcare sectors
  • Adhere to pre-implementation rollout strategy by collecting necessary agreements, holding stakeholder engagement, completing readiness assessments and pre-implementation surveys
  • Offer onsite training (direct or train-the-trainer) based on the requirements of the project deliverable
  • Provide ongoing support with any unresolved issues and/or implementation challenges
  • Continuous learning on new EMR systems and software updates as they become available
  • Coordinate with CMS/EMR vendor for any additional EMR training requirements
  • Supplements site technical resource, provides Q&A testing
  • Coordinate CMS/EMR vendor upgrades or patches and validate the work being done
  • Work with local user groups to confirm and document report receipt & acceptance as needed
  • Coordinate post implementation follow up to confirm acceptance/adoption of solution
  • Track and monitor implementations and log any issues in a common Portal
  • Provide lessons learned for future use and provide to Change Management Lead
  • Identify successes and challenges in adoption by monitoring feedback from users with regards to gaps/resistance in process; relay to the Change Management Lead/project team

EDUCATION & EXPERIENCE:
 
  • Minimum 2 year College Diploma
  • Minimum 3 years change management experience
  • Demonstrated experience in developing and delivering change management and adoption programs including communications, training and adoption strategies
  • Working in a human resource, communications or organizational development role
  • Knowledge of latest theories, models and strategies for assessing and leading organizational change, including documenting and design of business processes
  • Demonstrated ability to understand the inter-relationship between business processes and technology tools
  • Experience in defining the impact that change will have on staff/management/customers
  • Demonstrated knowledge of and experience in the healthcare sector
  • Strong interpersonal skills and the ability to handle difficult situations with tact and diplomacy
  • Strong skills with computer systems including all Microsoft offices tools
  • Demonstrated ability to quickly adapt and/or learn new system functionality and new technology as a user
  • Previous working knowledge of an EMR system (multiple EMR systems a strong asset)
  • Ability to manage time/heavy work load effectively
  • Strong analytic and decision making skills
  • Ability to work independently as well as within a team

  • Must be able to lift 30 pounds on occasion
OTHER CONDITIONS:
 
  • Must be able to travel within the Erie St Clair area on a regular basis
  • Must have dependable vehicle and valid drivers license
  • Must be able to work evenings and weekends, when required
  • Must be available to perform other duties as assigned
