We are looking for a great fit and can offer part time, full time, or locum as requested by the successful candidate.

This position offers a unique opportunity to make a difference in the lives of those impacted by homelessness, mental illness, substance use, poverty, complex and/or chronic health conditions. The Nurse Practitioner works as a member of an interdisciplinary care team of Physicians and Nurses. The clinical team, in turn, is supported by Social Workers, Physiotherapists, Dietitians, Outreach Workers, Community Developers and support staff. In wrapping services around the client, the Nurse Practitioner will be required to provide clinical support and expertise to other program areas within the Health Centre.

REQUIRED QUALIFICATIONS

  • Bachelor or Masters of Science in Nursing
  • Primary Health Care Nurse Practitioner Certificate
  • Registered Nurse, Extended Class in good standing with the Ontario College of Nurses
  • Minimum of 2 years primary care experience, preferably in a community health setting
  • Awareness and understanding of the social determinants of health
  • Commitment to the underlying principles of harm reduction
  • Ability to work with diverse communities with respect, sensitivity, creativity, innovation, cultural understanding, non-judgement and facilitation skills

PREFERRED SKILLS

  • Excellent written and oral communication skills
  • Experience with Electronic Medical Records (Telus Practice Solutions Suite currently in use)
  • Ability to maintain complete and accurate records in accordance with the Health Centre’s policies
  • French and other language skills are considered an asset in supporting the communities we serve
