Registered Nurse - Diabetes Educator

Temporary full-time contract to cover a planned leave of absence

The Health Centre seeks a Registered Nurse (RN) committed to providing high quality education and healthcare services to people living with diabetes, as well as people who are at risk of developing diabetes. The RN works as a member of a multidisciplinary team supported by physicians, nurse practitioners, registered dietitians, social workers, community development workers, and support staff. In this role, the RN will provide diabetes education counselling, community outreach and health promotion to individuals, families, and the community.

THE COMPENSATION PACKAGE

  • A starting salary range of $74,000 - $78,000
  • Monday to Friday - no weekends!

REQUIRED QUALIFICATIONS

  • Postsecondary diploma (RN) or degree (BScN) from a recognized college or university
  • Certified Diabetes Educator (CDE) through the Canadian Diabetes Educator Certification Board (CDECB) or working towards certification
  • Registration in good standing with the Ontario College of Nurses
  • Minimum 3 years of clinical diabetes experience, preferably in primary care
  • Experience developing and following diabetes care plans with MD/NP
  • Experience facilitating individual and group education sessions
  • Awareness and understanding of the social determinants of health, health promotion and adult education principles
  • Demonstrated ability to work with individuals from diverse backgrounds through an inclusive and anti-oppressive lens

PREFERRED SKILLS

  • Experience with Electronic Medical Records (Telus Practice Solutions Suite currently in use)
  • French and other language skills are considered an asset in supporting the communities we serve
